UPGC POLICIES


Assumption of Risk

Sports participation can be dangerous. You must recognize that participation in all activities offered at UPGC can result in minor and severe injuries, permanent paralysis, brain damage, or even death. A Waiver Release Form must be agreed to, signed and turned in before your child may participate in the first class.


Medical Emergencies

You must agree to authorize transportation of your child to a medical facility for treatment in the event of an accident or emergency, at your cost, and hold UPGC harmless in the management of such accident or emergency. You must agree to provide for all medical expenses which may be incurred by yourself or your child(ren) as a result of any injury while on our premises or while under our care. If at any time during your child's enrollment with UPGC said child becomes injured (on UPGC site or off), UPGC must have a doctor's note releasing the enrolled child to full activity before they are allowed to return to class.


Registration Fee

There is a $40 Annual Registration Fee per child that is non-refundable. Please be aware of this when you register for a class. The Annual Fee is valid from September 1st of one year to August 31st of the following year. Summer session for new registrations will be a prorated registration fee.


Payment Policies

ACH (Auto-debit from a checking or savings account) form is required for payments. The ACH must be filled out no later than the 2nd week of class and is available in the office, or requested via email. If payment is declined for any reason, the customer is responsible for any fees charged to UPGC due to the decline. In case of chargeback, all fees for services rendered must be repaid to UPGC. Without payment in full, the child cannot continue/enroll for future classes.


Cancellation or Changes

There is a $30 returned check fee/returned ACH. Each attempt to retrieve funds unsuccessfully will result in an additional $30 fee. If registering for a class after participation in a no-obligation trial class, the trial class will be charged as instructional time with remaining monthly tuition.


Make Up Policy

Make-ups must be scheduled and completed within 2 weeks of the missed class. If there is no available space in another class, a makeup class may be completed in an equivalent class, lower level class, or an Open Training pass may be offered. All make-ups must be completed within the current session or they are forfeited.


Tuition Payments

Monthly class tuition is paid in advance. An invoice will be sent after online enrollment via email so you can make an online payment from your checking account, or you can make payment in-person at our office. You may also mail a check payment to Ultimate Performance Gymnastics and Cheer, 4289 124th St., Chippewa Falls, WI 54729.


Tuition Due Date

Tuition is due within 3 business days of registration. Classes fill very quickly and payment secures a class spot for your child. Payment must be received no later than 3 business days of registration or your child may be placed on a waiting list and may lose their spot to a fully paid customer.


The UPGC Waiver Release Form must be signed and turned in along with paid tuition before your child may participate in the 1st class. After the 1st week of non-payment, your child will be dropped from the class, unless otherwise notified. Our school year sessions are for 9 months. No refunds are issued after the first week of the month, regardless of participation. Our summer sessions are 10 or 11 weeks, dependent on the year.


Parent Observation

We have an Open Door Policy. Parents are welcome to sit in the viewing area and observe their athlete. Please observe without distracting your athlete, this includes coaching your child during the class. Athletes need to be allowed to learn in a pressure-free environment. If a parent continues to distract their athlete, they may be removed from the lobby. Children in the lobby must be attended by a parent or guardian. NO HORSEPLAY WILL BE TOLERATED in or out of the gym. Safety rules must be followed by all patrons on site.


Drop Off, Pick Up Times

Drop off your class participant no more than 15 minutes prior to class. Pick up promptly after class has ended. You may leave your child during class if your account information is up to date and the child is in the Gym Gem (school-age) levels. Preschool parents must remain on site. If your child is enrolled in our KinderGym or Super K classes, please have an authorized adult on site during your child's class. In order to attend each session, your child must be registered and your account must be in good standing. You may not register for another class if there are outstanding bills on your account.


Parent Participation Classes

You and Me Kid must have an adult participating in the class with them. 3 Year Old KinderGym must have an adult remaining on site and may require parent participation as necessary. Parents must sign a waiver for themselves before participating and must stay off the equipment. Only one participating adult per student, per class. The adult must be actively involved in class with the child. No cell phones in the gym. No shoes in the gym.


Gymnastic Attire for Girls:

A form fitting one-piece leotard. Compression shorts are allowed. NO skirted leotards, they are a safety issue, and can get wrapped around the bar or the spotter's arm, and can cause injury.

Hair MUST be TIGHTLY SECURED and out of the athlete's face. The teacher will return the student to the parent to correct the hair if it is not secure. This is a SAFETY ISSUE! We sell ponytail holders for $0.25 in the office if you forget to bring one for class.

Bare feet.

NO JEWELRY OR WATCHES. Small stud earrings are fine.

Gymnastic Attire for Boys:

Elastic waist workout shorts. NO JEANS! Clothes must not have any buttons, zippers, or other items that might poke the participant or damage the equipment.

A non-baggy t-shirt.

Bare feet.

UPGC is not responsible for any items lost or misplaced in the gym during a class. A class instructor is not allowed to hold items for the child during class. Shoes and articles may be placed in designated areas. No gum is allowed during class.


15 day notice of Departure

If you decide (for any reason) to drop your child's class, you are required to fill out and submit a 15-day notice of departure form in our office. This may also be requested via email. The notice must be in writing and be submitted 15 days prior to departure. Any outstanding fees must be paid in full and tuition will be due during the 15 days prior to departure. The 15 days will begin on the day the form is submitted in person, in our office.

Parent Responsibilities

Parents and children who arrive at the gym early are to remain in the designated Observation area until the instructor signals the beginning of class. Children are not to run around in the Observation area and must be supervised when in the building and not in class.

Parents should accompany young children to the bathroom before class.

Only students enrolled will be allowed to be in the gym areas during classes. Siblings not enrolled in class are to remain with the parents.

Parents may remain in the Observation area during classes.

Plan to be early for class, and if you leave the building during their class time always be sure to be back to pick up on time. If you are going to be late for any reason you MUST call the gym (715) 861-7333 to notify us.

Children must remain inside the building until you come in to pick them up. Children are never allowed to wait outside the building doors.

If someone other than you is picking up your child please notify the front desk.

Help us with the traffic flow in and out of the parking lots. Enter slowly; always look before you begin backing in or out of the area. We all know how unpredictable little ones are.

HELP US KEEP A SAFE FACILITY FOR OUR FAMILIES. NOTIFY THE OFFICE IMMEDIATELY IF YOU SEE ANYTHING IN OUR AREA THAT IS SUSPICIOUS OR OUT OF THE NORM.


Behavior Warning

A warning will be given the 1st time inappropriate behavior is displayed by a student. The second time they will be asked to sit out for 5 minutes. If the inappropriate behavior continues then the student will be asked to leave the class and the parent/guardian will be informed.


Inclement Weather

Notification of class cancellations will be posted on our Facebook page. We do not necessarily close for inclement weather in response to local school closings, but rather the weather conditions affecting travel around class times. If you are concerned about weather or road conditions, we ask that you use your best judgment and schedule a make-up class if you feel you cannot make it to class safely.


Wellness/Illness

Inform us if your child has any physical problems or medical information that we should know about.

Talk to your instructor and let them know of any problems that could affect your child's safety during class.

Do not bring sick children to class. We do not want to expose other children or staff members to any unnecessary illness.


Injury and Safety Procedures

We take every precaution to provide a safe environment, but because we are a sports related activity with movement and motion, on occasion, accidents will happen. Should an injury occur in the gym during class the instructor will follow the Safety Procedures Manual provided by Ultimate Performance Gymnastics and Cheer.


Facility Cleanliness

Cleanliness is priority at Ultimate Performance Gymnastics and Cheer! We have high traffic areas throughout our gym! Mats and carpets are sanitized for health and safety standards on a daily and weekly schedule. All food and drinks other than water must remain inside the Observation area and outside of the main gym. Please report any spills to the front desk. Do not hesitate to report if the restrooms need attention.


Parking

Please use the parking spaces provided. Do not park along the building or in front of the door, blocking the sidewalk area. If you drop off a student at the front door, we ask you to watch for them to make their entrance before you leave. Enter and exit slowly/always look when you are backing up.


Communications

Regular updates can be found on our Facebook page, website, and gym bulletin board. We also send out newsletters by email. It is very important that we keep you informed on all activities, special programs, etc. happening in the gym. Get to know your child's teacher and visit with them briefly before or after class. They are more than happy to answer any questions or concerns that you may have about our program and activities. Contact us immediately about any concerns, so that we can be proactive! Our goal is to provide a positive athletic experience!
