Assumption of Risk
Sports participation can be dangerous. You must recognize that participation in all activities offered at UPGC
can result in minor and severe injuries, permanent paralysis, brain damage, or even death. A Waiver Release Form
must be agreed to, signed and turned in before your child may participate in the first class.
You must agree to authorize transportation of your child to a medical facility for treatment in the event of an
accident or emergency, at your cost, and hold UPGC harmless in the management of such accident or emergency. You
must agree to provide for all medical expenses which may be incurred by yourself or your child(ren) as a result
of any injury while on our premises or while under our care. If at any time during your child's enrollment with
UPGC said child becomes injured (on UPGC site or off), UPGC must have a doctor's note releasing the enrolled child
to full activity before they are allowed to return to class.
