At Ultimate Performance
Gymnastics we strive to be accommodating, while maintaining a safe, structured environment where all gymnasts can
learn and have fun! Please read these guidelines carefully and go over the relevant parts with your children, so
that they understand our rules and expectations (just like school)!
Medical History
and Waiver Release Forms:
A completed Registration Form with the Medical History/Waiver Release Form must be on file for each student enrolled
at Ultimate Performance Gymnastics LLC. The Waiver Release Form must be printed, signed and dated by both parents/guardians (required by our insurer) and renewed at the beginning
of each new year or at the time of enrollment. No child can participate in classes unless we have this form on

Tuition Payments:
1. Ultimate Performance Gymnastics accepts checks, cash (in-person), and credit card payments through Visa, MasterCard,
American Express, Discover, Diners Club, and JCB.
2. All checks for tuition may be made to Ultimate Performance Gymnastics.
3. On request we can do an electronic funds transfer (EFT) from your checking account.
4. Tuition is due by the first class of each session.
5. Higher tuitions can be paid in 2 payments, 1st payment by the first class of the session, 2nd payment in the
middle of the session. A specific date will be determined for each session
Make Up Policy:
1. One make-up per term (exception - classes that fall on a holiday).
2. Make-ups are limited (one per term) and must be scheduled in ADVANCE through the front desk.
3. Make-up can be for same level class or one level lower.
4. Pick up your child's Make-up Pass upon arrival and give to the instructor.
5. Missed classes cannot be used to discount tuition.
6. If you are unable to schedule a make-up class, you may request an open gym pass in lieu of a make-up.
Classes:
1. Ultimate Performance Gymnastics will make every effort to keep class instructors consistent. Though sometimes
emergencies do occur and a substitute instructor is required.
2. Ultimate Performance Gymnastics reserves the right to cancel a class if the enrollment is less than 3 students.
We will work with you to place the student in an alternative class of the same level.
Parents' Responsibilities:
1. Parents and children who arrive at the gym early are to remain in the designated Observation area until the
instructor signals the beginning of class. Children are not to run around in the Observation area and must be supervised
when in the building and not in class.
2. Parents should accompany young children to the bathroom before class.
3. Only students enrolled will be allowed to be in the gym areas during classes. Siblings not enrolled in class
are to remain with the parents.
4. Parents may wait in the Observation area during classes.
5. Plan to be early for class, and if you leave the building during their class time (which is perfectly fine!)
always be sure to be back to pick up on time. If you are going to be late for any reason you MUST call the gym
@ 715-832-3138 so that we will be able to notify your child, their instructor and the directors.
6. Children must remain inside the building until you come in to pick them up. Children are never allowed to wait
outside the building doors.
7. If someone other than you is picking up your child please notify the front desk.
8. Help us with the traffic flow in and out of the parking lots. Enter slowly; always look before you begin backing
in or out of the area. We all know how unpredictable little ones are.
9. HELP US KEEP A SAFE FACILITY FOR OUR FAMILIES. NOTIFY THE OFFICE IMMEDIATELY IF YOU SEE ANYTHING IN OUR AREA
THAT IS SUSPICIOUS OR OUT OF THE NORM.
Students' Responsibilities:
1. Students will treat instructors & fellow students with respect.
2. Students will listen carefully & follow directions.
3. Horseplay will not be tolerated.
4. Students will wait for their turn and not push ahead of another student in the class.
5. A warning will be given the 1st time inappropriate behavior is displayed by a student. The second time they
will be asked to sit out for 5 minutes. If the inappropriate behavior continues then the student will be asked
to leave the class and the parent/guardian will be informed.
Attire for Girls:
1. Requirements
are to wear a body leotard, no tights, only compression shorts are allowed, no lose or baggy shorts.
2. Students will work barefooted or wear gymnastic shoes.
3. Hair must be pulled away from the face and secured.
4. No jewelry should be worn into the gym. We can't assume responsibility for any items lost or misplaced in the
gym during class. A class instructor is not allowed to hold jewelry for the child during class.
Attire for Boys:
1. T-shirts tucked into sweat pants or shorts. No overly baggy pants or shorts, as it is a safety concern.
2. Students will work barefooted or wear gymnastic shoes.
- Shoes and articles may be
placed in designated areas.
- No gum is allowed during class.
Inclement Weather:
Notification of class cancellations will be posted on the website www.teamupgym.com and on the Facebook
page. Please
call to check when necessary. We do follow the Eau Claire & Chippewa Falls Public Schools Closing for inclement
weather.
Wellness/Illness
Policy:
1. Please inform us if your child has any physical problems or medical information that we should know about. Do
talk to your instructor and let them know of any problems that could affect your child's safety during class.
2. Please do not bring sick children to class. We do not want to expose other children or staff members to any
unnecessary illness.
Injury and Safety
Procedures:
Every precaution is taken to prevent injuries by providing qualified Certified Staff and USAG equipment and matting
recommended by our national program. But, because we are a sports related activity with movement and motion, on
occasion, accidents will happen. Should an injury occur in the gym during class the instructor will follow the
Safety Procedures Manual provided by Ultimate Performance Gymnastics LLC.
Parent Observation:
Parent observation is always available through the observation windows. When our staff invites you inside the gym
area for any classes, we must have siblings secured and under parent supervision. Programs and opportunities are
offered during the year when parents and friends may come into the gym for viewing and observation.
Facility Cleanliness:
1. Cleanliness
is priority at Ultimate Performance Gymnastics!
2. We have high traffic areas throughout our gym! Mats and carpets are sanitized for health and safety standards
on a daily and weekly schedule.
3. All food and drinks other than water must remain inside the Observation area and outside of the gym. Please
report any spills to the front desk.
4. Do not hesitate to report if the restrooms need attention.
Parking:
1. Please use the parking spaces provided. Do not park along the building or in front of the door, blocking the
sidewalk area.
2. If you drop off a student at the front door, we ask you to watch for them to make their entrance before you
leave.
3. Enter and exit slowly/always look when you are backing up.
Communications:
1. Monthly Newsletters are available online and also available at the front desk area.
2. It is very important that we keep you informed on all activities, special programs, etc. happening in the gym.
3. Get to know your teachers and do feel free to visit with them briefly before class. They are more than happy
to answer any questions or concerns that you may have about our program and activities.
